Do you have your child's official birth certificate?

Kid with BagYour child must have a certified birth certificate to attend school.

There are 3 ways to order a certified birth certificate:

You can only request a birth certificate for yourself or an immediate family member and others under special circumstances.

IN PERSON

Virginia Department of Motor Vehicles Customer service locations offer vital record service with proper identification. Danville’s location is: 126 Sandy Ct., Ste. C, Danville, VA. Cost is $14.  THIS IS THE QUICKEST OPTION!

ONLINE OR BY PHONE

Visit www.vdh.virginia.gov/vital-records/ to order a certified birth certificate through VitalCheck or call 877-572-6333 (press option 1) to place your request. Cost is $20.80. Expedited delivery charges may apply based on your selection.

BY MAIL

You may mail a completed Application for Certification of a Vital Record to the address listed below with a copy of valid identification included. Cost is $12 and will only be accepted in the form of a check or money order.

Are your child's shot records up to date?

Before your child is allowed to attend school, you must provide proof of all required immunizations. It is a good idea to complete your child's immunizations well before the first day of kindergarten. Contact your child's physician and ask for his/her records today.

If you have no insurance, the health department offers free immunizations for children. For more information or to set up an appointment, call Pittsylvania-Danville Health District in Danville at 766-9828 or in Chatham at 432-7232. Make sure you have any prior immunization records before you call.

Does your child need health insurance?

Call 1-855-242-8282 or visit www.famis.org to enroll in Family Access to Medical Insurance Security (FAMIS), a program that helps families provide health insurance for their children.

Eligibility is based on income.
